CANCELLATION & MISSED APPOINTMENT POLICIES
Thank you for choosing PhysioHealth to provide your health care. We consider it an honor and privilege to be of service to you and look forward to a long and mutually satisfying relationship. We do understand that there may be extenuating circumstances; however, we request that any cancellation of your appointment be made 24 hours in advance. We want you to know that we value your time and hope that you value our time, too. Missed appointments or appointments cancelled less than 24 hours in advance affect our community of both patients and health care practitioners. A missed appointment means the doctor or therapist could not see other patients who might be needing care that day. So, we have created a cancellation and missed appointment policy.
POLICY
You, not your insurance company, will be charged 50% of your office visit for a missed appointment or appointments not cancelled at least 24 hours in advance or your scheduled visit. The maximum charge for a missed appoint will not exceed $80.00.
REMINDER CALLS
We provide reminder calls the day before your appointment as a courtesy. You are responsible for remembering your scheduled appointment. Stating that you did not receive a reminder call or that the call was made after the 24 hour deadline does not make your missed or cancelled appointment an exception.
